Cancer treatment & Management Fund for poor & J&K aid trust (MAT)

ELIGIBILITY:

The benefit of this scheme is only for poor patients of resident of J&K UT, whose family income does not exceed Rs. 20,000/- per month shall be eligible for receiving Financial assistance under the fund.

ELIGIBILITY:

The applicants shall belong to SC/ST whose family income does not exceed 2,50,000 per annum are eligible.

ELIGIBILITY:

The applicant belongs to (BPL) suffering from life threatening disease.

Cancer treatment and management including surgery, Chemotherapy, Radiotherapy or a combination of theses regimens.

The applicant suggering from major ailment which need surgery such as kidney, heart, liver, Cancer brain or any other life threatening disease include organ transplant and spinal surgery.

The applicant suffering from

i.    Cardiology

ii.    Cancer

iii.    Urology/Nephrology/Gastroenterology

iv.    Orthopaedics

v.     Neurosurgery

vi.    Endocrinology

vii.     Mental Illness

viii.    Drugs

ix.    Investigation.

Income of the patient/family and estimated cost for treatment/Management and Financial Assistance shall not exceed 60% of the estimated cost of the treatment.

100% of the estimated cost of the surgery will be directly released to the concerned Hospital with a maximum ceiling limit in the form of crossed cheque/DD

1.        Heart Surgery      1.25 Lac

2.        Kidney Surgery/Dialysis = 3.50 Lac

3.        Cancer Surgery/Chemotherapy/Radiotherapy  =1.75

4.        Brain Surgery = 1.50 Lac

5.        Kidney/Organ Transplant  = 3.50 Lac

6.        Spinal Surgery = 1.00 Lac

7.        Other Threatening disease = 1.00 Lac

Financial Assistance is release in the form of one till grant only.

·          No upper limit has been fixed for availing Financial Assistance.

·          No Central/State/PSU Employees are eligible for Financial Assistant.

·          No common nature and disease for which treatment is available free of cost under other Health schemes

·          No grant is released to patients seeking treatment private hospitals.

·          *Re-imbursement of medical Expenditure already incurred for treatment/operation.

Applicant submit the Application to the Dy. Commissioner alongwith Estimated cost taken from (HOD) duly countersigned by the Director/Principal of the institute where the patient is registered.

Documents Required:

1. Aadhar No. of Patient.
2. Residential Proof.
3. Income certificate issued by concern Tehsil.
4. Medical certificate of the Patient from the HOD of the concerned specialty of the institute, Medical Hospital duly countersigned by the Director/Principal.

The applicant shall apply on prescribed form duly certified by the medical Supdtt. Of the concerned hospital alongwith

i.    Caste Certificate

ii.    Income Certificate

iii.    Ration Card

iv.    Estimate cost of the surgery

And recommended and forward by the member of General body of Dr. Ambedkar foundation/Local sitting member of Parliament (Lok Sabha or Rajya Sabha) OR District magistrate and Collector/ Deputy Commissioner concerned District. Finally submitted to the DIRECTOR DR. AMBEDKAR FOUNDATION 15 JANPATH NEW DELHI atleast 15 days before the date of surgery.

The applicant enclosed format with medical report duly signed/stamped by the treating doctor and Head of department countersigned by medical Supdtt. In original. Income of patient/parent. Ration card alongwith its cover page of all the family members.
